Expanding care services within local communities
We want to provide as much care as close to where people live as possible.
Much of our 'Out of Hospital' work happens within each of our Places and through their respective Partnership arrangements.
A key focus of this work is to support people within our communities to stay well and make choices that are important to them in improving and maintaining their health and wellbeing. We aim to deliver care as close to people’s homes as safely as possible and to ensure that when hospital care is needed there is sufficient community capacity available to support safe and speedy discharge.
There are clear linkages between the Out of Hospital approach and a range of other national priorities and ICS Programmes: Population Health Management, Personalisation, Primary Care, Ageing Well, Digital, Innovation and Workforce Development.
