Sheffield is a city and metropolitan borough in South Yorkshire. It is one of the eight largest regional English cities and is the second-largest city in the Yorkshire and the Humber region.

  • The Sheffield Health and Care Partnership (HCP) is an alliance of health and social care organisations that work together to focus on key issues that can only be addressed together.
  • The Sheffield HCP partnership vision over the next ten years is for our health and care services to be joined up, and seamless; to reduce and remove inequalities in a way that involves people, their experiences and our communities at the centre.

New initiatives:

  • Peak Health Coaching have designed a person-centred approaches virtual workshop that is aimed to be delivered to over 500 health and care staff across Sheffield to create ‘a flourishing and thriving Sheffield by developing our people in a joined up way to deliver holistic and person-centred and integrated care’ – Shaping Sheffield, 2019.   • Leading Sheffield is a leadership development experience that offers some tools and techniques for adapting to and applying system leadership. The programme  gives the opportunity to work with colleagues from other organisations through addressing a specific challenge affecting the people in their community. These challenges develop relationships and agree ways of working across the partners of the HCP and other relevant organisations e.g., policing and education.

  • The Sheffield Neurodevelopment Transformation programme is being undertaken by health, care and voluntary organisations across Sheffield to improve the neurodevelopment services provided, to better meet the needs of children with neurodevelopmental conditions through working with families and carers as well as the children and young people themselves. System Plans and Developments:

  • Health and social care organisations in Sheffield have been working together during the COVID-19 pandemic to solve the complex challenges presented throughout the outbreak. 
  • A number of cross-organisational, community groups have been set up to work collectively and develop a consistent approach to readdressing racial inequalities in our health and care system.
