  Health and Care Integration

The Health and Care Integration Programme is continuing to build on the networks and partnerships gathered over the past few years, strengthening our foundations to support the growth of our ‘One Workforce’ of the future. A key step to moving forward, is creating the right conditions for a ‘Culture of Integration’ to discover, develop, embed and sustain. To do this we are being guided by a cross section of principles:

To ensure that we deliver a truly Integrated model we are exploring an approach described in the ‘Connected Communities’ Connected Community: Discovering the Health, Wealth, and Power of Neighborhoods. Cormac Russell & John McKnight | Health & Social Work | Oxford Academic (oup.com)  The power in this methodology is, being part of the solutions with communities by understanding their needs. Phase 1 – ‘Discovery’ has already begun, starting with our colleagues in the Primary Care Teams, discovering the great work being undertaken and what that means for enhanced roles and the future ‘One Workforce’.
