The Health and Social Care Integration Hub currently has six objectives from 2023/2024.

These include:

  • Social Care Placement Expansion – Supporting the Growing for the future workforce workstream by facilitating the growth of placements by 20% in line with the HEE LEAP/CPEP ambitions. Improving educational opportunities contributing to reducing inequalities within the South Yorkshire area. Reported through QSIR.
     
  • Support and facilitate the development and embedment, by 2024, of the innovative Enhanced Social Care Skills Programme project for the social care workforce with health cost benefits. Including Promoting partnership and aligned to HEE’s New Ways of Working ambitions. Reported through QSIR.
     
  • H&SC Academies – Aligning with workforce NHS Priorities, supporting the development of the place-based academy modelling, utilising provider collaborative by April 2024. Improving educational and employment opportunities reducing inequalities within local communities.
     
  • Continue to support digital activity into social care such as ECHO Digitalising Social Care – In alignment with the transformational aims of NHSE Digitalising Social Care and Primary Care, the provision of awareness training with workforce/providers regarding Electronic Health Records (NHSE national target is 80% of CQC registered providers by March 2024) and registration of Unpaid Carers. Preventing health escalations, personalising care and improving experience.
     
  • NHS Commitment to Carers – Development and installation of the Unpaid Carer strategic approach by October 2023 through collaboration with carers/partners. Highlighted areas of focus: Workforce, Health and Well Being of working carers, Carers Forum (Carer Capacity), Data, Partnership engagement, Communication/media, Funding, Primary Care and Carer.
     
  • Primary Care – Strengthen the partnership working between primary care and social care and harnessing the joint workforce in new ways. in turn lead to improvements in health inequalities and population health. Focus areas, ARRS, Placement expansion and Carer support.
